BOSTON, JANUARY 2022 – The Heart of a Giant Foundation today announced it has been awarded a $5,000 grant from CVS Health, which will help reduce overall health care costs, and improve health outcomes for the most vulnerable residents in the Dorchester, Mattapan and Brockton communities. Through this grant, HGF will be able to provide health education and nursing support to participants in the 2HC program.

“We are very grateful to CVS Health.  Not only did they make a generous cash donation, they have also gifted us with 60 blood pressure self-monitors, valued at $3600.  These devices will form part of the packs that we give to participants in our Healthy Hearts program.” Somaneh Bouba Dieme, Co-Founder and President of the Heart of a Giant Foundation.

With nearly 10,000 local touchpoints in communities across the country, CVS Health is delivering on its purpose of helping people on their path to better health through strategic community investments. The company supports communities in a variety of ways from providing innovative new services and transformative programs that help manage chronic conditions, to making health care more accessible, more affordable and simply better.

“As part of our commitment to address the social determinants of health, and to break down the barriers for people to access quality affordable health care, we’re proud to support the Heart of a Giant Foundation’s work in Boston for those who need it most,” said Eileen Howard Boone, SVP of Corporate Social Responsibility and Philanthropy for CVS Health, and President of the CVS Health and Aetna Foundations.

The Heart of a Giant Foundation will shortly be launching its flagship project, the Healthy Hearts Community Program (2HC). This is a community-based, technology driven intervention to help manage and prevent hypertension.  The program offers clinical care, health education and lifestyle support to help people improve their heart health.

About the Heart of a Giant Foundation

The Heart of a Giant Foundation was founded by Somaneh Bouba Diemé, an engineer and entrepreneur from Mali and Senegal.  In 2016 Bouba was diagnosed with a congenital heart condition known as left ventricular non-compaction cardiomyopathy. Since October 2016, Bouba has been on the waiting list for a heart transplant.  While he waits for a donor’s heart, he lives with an LVAD Heartmate3 pump, which was implanted in his heart in early 2017.

Following his experience with heart disease, Bouba became intently committed to raising awareness about hypertension and other cardiac conditions, particularly in predominantly BIPOC communities.  As a result, he founded the Heart of a Giant Foundation in 2020 as a nonprofit organisation with a mission to improve health outcomes for people at risk of heart disease.
